The Family Registration allows for up to two adults and up to five (5) playing youth/juvenile club members. Children being registered, must intend to play within our team structures. A youth/juvenile player is defined as an individual under 18 years old.
On completion of this membership application form, on behalf of the named juvenile applicant, I as the parent/guardian/member:
(a) Hereby apply for membership of Laochra Óg Hurling & Camogie Club and membership of Cumann Lúthchleas Gael (Gaelic Athletic Association) and of Cumann Camógaíochta (Camogie Association). I subscribe to and undertake to further the aims and objectives of Laochra Óg Hurling & Camogie Club and of Cumann Lúthchleas Gael and Cumann Camógaíochta to abide by its rules and regulations.
(b) If registering any player under 18 years of age, you need to; Read and accept the terms and conditions set out in “Our Games Our Code - Code of Best Practice in Youth Sport", Read and accept the terms and conditions set out in “Code of Behaviour Underage (2018 Edition)”. Complete the “Juvenile Parental Consent Form” and provide details of any relevant medical condition / medication.
(c) Give permission for the juvenile to participate in Laochra Óg training, games, and related activities.
(d) Consent to mobile phone number and e-mail address listed overleaf being used by the club to facilitate ease and efficiency of communications, regarding club related activities i.e. training, matches, fundraising, events etc. It is club policy that this information be sent to the parent / guardian of the juvenile player rather than directly to the juvenile players.
(e) Give permission for my juvenile to be photographed / filmed in relation to matches / coaching to be used for promotional activities i.e. local media, club website etc. Such photographs will adhere to the G.A.A. and the CAMOGIE ASSOCIATION Guidelines for use of photography and filming. Should you have an objection to the juvenile being photographed / filmed, do not accept the terms and conditions and contact a club trainer for clarification.
(f) Consent to team mentors administrating first aid, where appropriate, in the event of the juvenile requiring same.
(g) Provide herewith appropriate membership fee as determined by the club

We/I consent to the above application and to undertakings given by me or the applicant.
· We/I understand the personal data on this form will be used by the Club, GAA and or Camogie Association for the contractual purpose of registering (or re-registering) and maintaining mine or the applicant’s membership.
· We/I understand that the personal data will be retained by the Club, GAA or Camogie Association for such period as mine or the applicant’s membership subsists.
· We/I understand that I can resign mine or the applicant’s membership by writing to the Club, GAA or Camogie Association and then mine or their Personal Data will then be erased.
· We/I understand that mine or the applicant’s personal data will also be used for administrative purposes only to maintain mine or their membership including club and team administration, registrations, team sheets, referee reports, disciplinary matters, injury reports, transfers, sanctions, permits and for statistical purposes.
· We/I understand that if I do not provide mine or the applicant’s personal data, mine or their membership cannot be registered with the Club, GAA or Camogie Association.
I have read the important data protection information on this form and have given my consent for my information to be used as follows:
(1) To provide me with updates regarding Club activities such as games, training, meetings and club events.
(2) To provide me with details of Club fundraising activities including lotto, social occasions, ticket sales etc.
(3) I am aware that my child’s photograph or video image may be taken whilst attending or participating in games or activities connected with the Club and I consent to it being used by the Club for items like match programmes, year books, match reports, event reports or on the Club website or social media channels.

The purpose for processing your personal data is that it is necessary for the performance of a contract to register and maintain your membership with the Club, GAA. or Camogie Association.

Will anyone else receive a copy of my personal data?
Your personal data can be accessed by certain members of the County Committees, Provincial Councils and the Central Council of the Gaelic Athletic Association for administrative purposes. This will be done in accordance with our data protection policy only.
In the event of an injury or insurance claim, details of your claim which will include your personal data will be passed to the GAA’s Insurance underwriters, Willis Insurance, Elm Park, Merrion Road, Dublin 4, Ireland.

Your data will be stored electronically on the GAA Membership Database or the Camogie Association Membership Database, both which are provided by Servasport Ltd, 11th Floor, Causeway Tower, 9A James Street South, Belfast, BT2 8DN.
Who is Servasport Limited?
Servasport Limited is a “data processor” who hosts the database on which your information is stored. We have a contract in place with Servasport Limited to ensure your personal data is stored safely and securely.
How long will your personal data be stored for?
Your personal data will be held for the duration of your membership and it will be deleted by us if you resign your membership, or you are expelled in accordance with the Official Guide. However, we may retain your personal data after your membership ceases if we decide that it is strictly necessary to do so in the circumstances in accordance with our data retention policy.
